Transition of Care of U.S. Veteran patients

Options for Care

Despite our Veterans program ending, Dr. Stearn and St John’s Cognitive Health continue to serve U.S. Veterans as patients.

The following options are available for U.S. Veterans to receive care:
• St John’s Cognitive Health is recognized as a Veteran’s Choice provider for eligible Veterans
• St John’s Cognitive Health bills as a courtesy to any medical insurance, and we are contracted with many private insurers as well as Medicare and Wyoming and Idaho Medicaid. For an updated list of in-network insurance providers, click here
• For Veterans transitioning cognitive health care to their VA Primary Care Team, we are happy to provide consultation to your primary care provider and ensure they have full access to your medical records from our office. Please contact us via phone at 307 739 7434

St. John's Institute for Cognitive Health

Program Background

Traumatic brain injury, PTSD and neurodegenerative disorders cause memory problems that are prevalent among veterans. St. John’s Cognitive Health and University of Utah Center for Alzheimer’s Care, Imaging and Research collaborated from June 2011 to March 2016 to offer an innovative clinical program that provides diagnostic and proactive memory care to United States Veterans. This program was sponsored by the U.S. Department of Veterans Affairs Office of Rural Health.
