FREE Memory & Cognitive Health Evaluations for United States Veterans

For a limited time, United States Veterans have access to FREE memory and cognitive health evaluations conducted by medical professionals from St. John’s Institute for Cognitive Health. Veterans and Veterans’ advocates are encouraged to contact the Institute to schedule an appointment and learn more about this unique program: phone 307 739 7434.

Services Offered

The goal of this program is to identify United States Veterans suffering from memory loss and provide care and/or referral assistance. Services offered include:

  • Cognitive impairment or memory loss assessments
  • Neuropsychological testing as needed
  • Medical referrals as needed
  • Supplemental diagnostic testing as needed, including lab work, x-rays, CT and PET scans, MRIs and sleep studies (fees may apply)
  • Proactive social work services, including support and resources
  • Personalized patient education
  • Care coordination with patient’s primary care provider
  • Information regarding regional services for veterans
  • Screenings available in Jackson, Afton, Idaho Falls and Pocatello

Program Background

Traumatic brain injury, PTSD and neurodegenerative disorders cause memory problems that are prevalent among veterans. St. John’s Institute for Cognitive Health and University of Utah Center for Alzheimer’s Care, Imaging and Research have collaborated to offer an innovative clinical program that provides diagnostic and proactive memory care to United States Veterans. This program is sponsored by the U.S. Department of Veterans Affairs Office of Rural Health.
